Patient Information & Forms

New Patients are always welcome. To schedule an appointment, please call (830) 626 5551 and select option 2. Please have your insurance card avalible when you call to schedule.

Our staff is happy to assist you in any way and every effort will be made to accommodate your schedule.

When calling for an appointment, if we will be billing insurance for your visit, please have your insurance card available. We take your insurance information over the phone so that your medical benefits can be verified prior to your visit. We do collect all co-payments, deductibles and co-insurance at the time of service.

All new patients (and established patients who have not been to our office within the past year) need to complete the Patient Information forms.

Please arrive at least 15 minutes early for your appointment and be sure to bring a current insurance card and current identification with you.

Office Hours
Our office is open to serve you Monday through Friday from 8:00 am until 5:00 pm.

Canceling Appointments
If you are unable to keep a scheduled appointment, please notify the office as early as possible by calling (830) 626 5551 and select option 2. This appointment time has been reserved for you, please cancel or reschedule 24 hours in advance if possible.

Payment Information
As a service to patients, our office will bill the patient’s insurance (both primary and secondary) for services rendered.

Co-payments and payments for treatment not covered by insurance are due at time of service.Charges, co-payments, co-insurance and deductibles are required at the time of the visit. We accept cash, check, Visa/MasterCardand Discover.
