When making an appointment, please have your current insurance information available. We take your insurance information over the phone so that your medical benefits can be verified before your visit. Click here for a list of accepted insurance plans. We collect all co-payments, deductibles, and co-insurance at the time of service. As a service to patients, our office will bill the patient’s insurance (both primary and secondary) for services rendered.
